Entry must be completed and submitted within 14 days after the Visit Date or by Januif the receipt date is Januor later to be eligible. Enter the store #, TC # and Visit Date located on the front of your receipt along with your complete name, mailing address, phone number and age in the appropriate fields, complete the survey, and click the “submit” button. Retain your receipt and visit within 14 days after the Visit Date printed on the front of your receipt. Not all receipts may contain a survey invitation. Make a purchase (of any amount) at a participating Walmart Canada retail location between Februand Januand look on your cash register receipt (“receipt”) for a survey invitation.
